- A completed application form—signed and dated
- A copy of your commercial driver's license
- 2017 Personal Federal Tax Return
- 2017 Business Federal Tax Return—only if filed separately from personal tax return
- Bank statements (past 3 months)
- Copy of the truck title (or pink slip)—only if retrofitting or scrapping existing truck
- Bill of sale (if purchasing a new truck) or quote/invoice (if retrofitting existing truck)

SCRAPPING BONUS
If you’re planning to scrap your non-compliant truck, a $6,000 scrapping bonus is still available to eligible loan applicants. To be eligible, you must first apply for a Clean Truck Fund loan for a replacement truck from either Opportunity Fund or Harborstone Credit Union. Opportunity Fund or Dolly Small.
In addition, beginning Jan. 1, 2019, all trucks serving our international container terminals must have a registered RFID tag AND have a 2007 or newer engine or certified equivalent emission control systems. Regardless of where you are in the financing or retrofitting process, you must have a compliant truck to enter the terminals. No exceptions.
